FORM FOR RESERVATION AND OTHER INFORMATION
In order to register your child as a
participant in the camp, the following information must be sent to the St.
Lazar’s Camp email address:
-Name
-Last name
-Date of birth
-Home address
-Telephone
-Email (of the child, if has one, and
parents)
-Names of parents
-Name of parish child belongs to
Compensation for each participant in the
camp is $150.
